Joshua Dean Lewis, “Forever Sweet Sixteen,” passed away Saturday, April 9, 2005. He was preceded in death by his great-great-grandmother Leota B. Holt ; uncle Butch W. Phillips; great-grandfathers Jim Miller and Floyd Mayfield.
